ADDITIONAL TERMS AND CONDITION a) Winning Bidder shall conduct a quarterly preventive maintenance service, inspection and operational check-up of the Equipment during the CONTRACTOR’s regular working hours (9am-5pm), Mondays to Fridays. b) Winning Bidder shall conduct a semi-annual (on the third quarter) general cleaning of the Equipment (cleaning solution shall be free of charge) the date and time of which shall be at the request of PAGCOR. c) Winning Bidder shall attend to service calls for adjustment, re-programming or repair of the Equipment arising from normal working operations and ordinary prudent uses of the Equipment. d) In case of critical service calls, Winning Bidder shall within two (2) hours upon receipt of notice immediately visit the site to diagnose and remedy the problem. e) Winning Bidder shall provide free subscription to remote maintenance service involving free ordinary adjustment/ repairs done on the IPPBX by means of remote service access by the Winning Bidder e.g. system diagnosis, trunk and tie line evaluation, etc. Since all ordinary adjustment/repairs done remotely shall be free of charge. f) Winning Bidder must ensure that the Service Level Agreement (“SLA”) is met or exceeded. g) Winning Bidder must monitor the overall performance of the Preventive and Remedial Maintenance contract delivery. h) Winning Bidder must review performance report with PAGCOR authorized representative on a regular basis. i) Winning Bidder must identify and implement with the PAGCOR appropriate improvement(s) in the delivery of the Preventive and Remedial Maintenance contract. j) Winning Bidder must provide services free of charge to PAGCOR for the transfer, relocation, installation, testing and commissioning of any CISCO IP PBX equipment currently installed at PAGCOR Corporate Main/Annex to PAGCOR Imus DR Site or at any PAGCOR location site within Metro Manila at the option of PAGCOR during the maintenance agreement and without voiding its warranty. k) Winning Bidder must agree with the following maintenance timeline and terms of payment: Maintenance Milestone Description Delivery of Periodic Maintenance % Payment of Contract Price 1st Quarter Submission of Service Renewal Kit / Document and First Preventive Maintenance Within fifteen (15) calendar days from receipt of Notice to Proceed 40% 2nd Quarter Second Quarterly Preventive Maintenance Within 3 months after the end of 1st Quarter 20% 3rd Quarter Semi-annual Preventive Maintenance& General Cleaning Within 3 months after the en...
