AMICABLE, CHESTERFIELD, AND CONCERNED CITIZENS. Any information about the employee may be included in the personnel file, including without limitation information regarding disciplinary action, such as client complaints, warnings, placements on probation status, and formal evaluation reports prepared by an Employer shall be placed in the employee’s personnel file and a copy shall be made available to the employee upon the employee’s written request. Each Employer shall allow the employeeand /or his/her representative (if the employee so authorizes in writing) to examine the employee’s permanent personnel file maintained in an Employer office, at a mutually agreeable time and date; files must be made available within five (5) business days of receipt of a written request. Employee’s who have a reasonable dispute with information in their personnel file may submit written comments no more than two
