Application for Permission to Convert. Prior to Licensee converting Licensee's Facilities on any Utility Pole, Licensee shall make written and electronic application to Central Xxxxxx and the Joint Owner, if any. Such application may be made for up to 125 poles at a time, or as otherwise agreed to by the Parties for major conversion projects. Licensee’s application shall be in the form attached hereto as Exhibit A-1 and Exhibit A-2. The non-refundable application fee of $8.18 per pole is waived as part of this Agreement. A Post- Construction Survey fee of $6.50 per pole shall accompany the application. Additionally, Licensee shall submit a strand map in digital format, CAD or GIS formatted in NAD 83 coordinates along with its application. Map contents should include a standardized GIS data set that includes the following information: Pole numbers, locations (Geographic Latitude and Longitude of the Attachment location), light sizes, light types and sequence of the proposed route for the Attachments. Licensee shall submit to Central Xxxxxx the detailed specifications and scope of work for any proposed LED installation. Detailed specifications must include size, type, weight and wattage. Licensee shall attach to each such application the Application Attachment Sheet, which shall be in the form issued by Central Xxxxxx and provided to the Licensee in electronic Excel format attached hereto as Exhibit A-2 with the columns A through G, completely filled out for each Pole together with name of the Licensee and the sheet number. Central Xxxxxx shall promptly review each application submitted by Licensee for completeness, and notify Licensee of any deficiencies found during such review. Central Xxxxxx shall process an application within thirty (30) Business Days after receipt of a complete application. An application will be considered complete only upon Central Xxxxxx'x receipt of the information detailed in this paragraph, along with a completed electronic and written application, the Application Attachment Sheet and the associated fees required by Central Xxxxxx.
