Applications for CIILB Days. i. Written Application. A CIILB member or a representative authorized by and acting for a CIILB member may apply for CIILB days. The written application and shall be supported by a healthcare provider’s statement stating the need for CIILB leave and describing the member's inability to work and the number of CIILB days requested.
