Applying for Days. a. Application for sick leave bank days will be made to the chairperson of the Sick Leave Bank Committee on a Sick Leave Bank Request Form. b. To be eligible for sick leave bank utilization, an employee must have exhausted all available sick leaves, comp time and vacation before sick bank days will be granted. c. The employee must have physician's statement when applying for days from the sick leave bank.
