Architect/Engineer Fee Statement. (Also see 1.) The Project Architect/Engineer shall use the OPFC format for the Statement for Architectural/Engineering Services, including Exhibits A and B, for all fee statements submitted to OFPC. The format is included in the Good Faith Effort Program Guidelines for Professional Services Contract referred to in 1, above. The Project Architect/Engineer shall enter the format, including Exhibits A and B, on a word processor for submission under the Project Architect/Engineer's letterhead stationery. The format, except for Exhibits A and B, may be modified only with the Project Manager's approval.
