Board Certified. The Physician is, and during the Term will be, Board Certified in Anesthesiology; or is pursuing board certification.
Board Certified. If the Contractor is board certified as Diplomate by the American Board of Psychiatry and Neurology for the psychiatrist series or in Addiction Medicine by the American Board of Preventive Medicine for the Addiction Medicine Series, Contractor shall receive a differential of 5% per hour for actual hours worked up to eighty (80) hours per pay period, excluding vacation and sick leave, in a pay period for professional activities required by the Department.

  • Governing Plan Document Your option is subject to all the provisions of the Plan, the provisions of which are hereby made a part of your option, and is further subject to all interpretations, amendments, rules and regulations, which may from time to time be promulgated and adopted pursuant to the Plan.
