Physician Qualifications. Seller Parties shall make reasonable efforts to ensure that each physician among the Clinical Providers at all times during the term of this Agreement meets the following criteria: (i) is qualified and duly licensed to practice medicine in the State of Georgia; (ii) holds participating provider status in the Medicare program; (iii) possesses customary narcotics and controlled substances numbers and licenses as required by all federal, state and local laws and regulations to prescribe, dispense and administer narcotics of a like nature to those prescribed, dispensed and administered at the Centers prior to the Effective Date; (iv) complies with applicable state medical licensing requirements pertaining to continuing medical education; (v) complies with any other reasonable requirements as set forth in Buyer’s policies and procedures, rules and regulations and made known to Seller Parties; and (vi) is appropriately credentialed and authorized to seek reimbursement under the Payor Contracts for services rendered.

Physician Qualifications. While this Agreement is in effect, Physician shall remain duly licensed and qualified as a physician under law and as required by other boards of authority. Physician's license will be clear and active with no history of discipline, exclusion, or sanction on file. Physician must be certified or credentialed through a recognized certifying or credentialing organization or by the Facility's legal or compliance departments. Documentary support for credentialing may include: Curriculum Vitae; Date of birth, driver's license number, and social security number; Copy of all valid licenses and permits; and Current certificate of insurance (evidence of coverage).
Physician Qualifications. (a) Section 3.23(a) of the Company Disclosure Letter lists (i) all of the physicians, by name, type of professional license, specialty and board certification (“Physicians”) and (ii) all of the non-physician providers, by name, type of professional license and area of practice (“Non-Physician Provider”) (collectively, Physicians and Non-Physician Providers shall be referred to as “Company Providers”) employed by Southwest Medical Associates, Inc. (“SMA”), a wholly owned Subsidiary of the Company, or other Company Facilities, as of the date hereof.
