Calculate Final Employment Outcome. (a) Overview: A SSN for a Population Member may be missing at the time of Randomization. To account for the fact that some Treatment Population and Control Population Members may not have a valid SSN at the time of Randomization, DOCCS Research and NYSDOL will divide the Population Employment Outcome by the overall percentage of Treatment and Control Population Members for whom a SSN was available at the time of Randomization.

  • Form B - Contractor’s Annual Employment Report Throughout the term of the Contract by May 15th of each year the Contractor agrees to report the following information to the State Agency awarding the Contract, or if the Contractor has provided Contract Employees pursuant to an OGS centralized Contract, such report must be made to the State Agency purchasing from such Contract. Any employee issued a uniform(s), who is leaving employment, will be required to turn in his/her uniform(s) prior to receiving his/her last check. The District will establish procedures for cleaning and maintaining rented uniform(s).
