Certificates of Insurance and Endorsements Requirements. The Certificate of Insurance shall be prepared according to the following instructions: The following address must appear in the Certificate Holder section: City of Cedar Rapids Public WorksCity Services Building 000 00xx Xxx. SW Cedar Rapids, IA 52404 The Project name and Contract Number being covered must appear in the certificate description area. All companies affording coverage must be clearly listed. All policy numbers must be clearly listed. Current policy dates and limits must be clearly indicated. If the professional liability coverage is written on a claims-made policy form, the certificate of insurance must clearly state coverage is claims-made. Furthermore, a Cancellation and Materials Change endorsement or a signed letter on the agent’s letterhead, from the insured’s insurance agent, confirming that the agent will provide notice that a cancellation, non-renewal, or material change is made to the policy shall be provided. The following endorsements are required to be included with the Certificate of Insurance to evidence that the policy has been endorsed: City of Cedar Rapids, Iowa ADDITIONAL INSURED ENDORSEMENT The City of Cedar Rapids, Iowa, including all its elected and appointed officials, all its employees and volunteers, all its boards, commissions and/or authorities and their board members, employees, and volunteers, are included as additional insureds with respect to liability arising out the Insured’s work and/or Services performed for the City of Cedar Rapids, Iowa.   • Certificate of Insurance/Endorsements A certificate of insurance from an insurer with a Best's rating of no less than A- indicating compliance with the required coverages has been received by the Department’s Contracts Management Bureau, X.X. Xxx 000000, Xxxxxx, XX 00000. Contractor must notify the Department immediately of any material change in insurance coverage, such as changes in limits, coverages, change in status of policy, etc. The Department reserves the right to require complete copies of insurance policies at all times.

  • Certificates of Insurance Prior to commencing under this Contract, Supplier must furnish to Sourcewell a certificate of insurance, as evidence of the insurance required under this Contract.   • Certificates of Insurance/Notices The Contractor shall provide OGS with a Certificate or Certificates of Insurance, in a form satisfactory to OGS as detailed below (e.g., an XXXXX certificate), after renewal or upon request. Certificates of Insurance shall:  Be in the form acceptable to OGS and in accordance with the New York State Insurance Law (e.g., an XXXXX certificate);  Disclose any deductible, self-insured retention, aggregate limit or exclusion to the policy that materially changes the coverage required by this Contract;  Refer to this Contract by award number;  Be signed by an authorized representative of the referenced insurance carriers; and  Contain the following language in the Description of Operations / Locations / Vehicles section: The People of the State of New York, the New York State Office of General Services, any entity authorized by law or regulation to use the Contract and their officers, agents, and employees are included as an additional insured on endorsement CG 20 10 11 85 (or endorsements that provide equivalent coverage, such as the combination of CG 20 10 04 13 (covering ongoing operations) and CG 20 37 04 13 (covering completed operations)), and General liability coverage is provided on the current edition of Commercial General Liability Coverage Form CG 00 01 (or a form that provides equivalent coverage). Additional insured protection afforded is on a primary and non-contributory basis. A waiver of subrogation is granted in favor of the additional insureds. Only original documents (certificates of insurance and any endorsements and other attachments) or electronic versions of the same that can be directly traced back to the insurer, agent or broker via e-mail distribution or similar means will be accepted. OGS generally requires Contractors to submit only certificates of insurance and additional insured endorsements, although OGS reserves the right to request other proof of insurance. Contractors are requested to refrain from submitting entire insurance policies, unless specifically requested by OGS. If an entire insurance policy is submitted but not requested, OGS shall not be obligated to review and shall not be chargeable with knowledge of its contents. In addition, submission of an entire insurance policy not requested by OGS does not constitute proof of compliance with the insurance requirements and does not discharge Contractors from submitting the requested insurance documentation.

  • Other Insurance Requirements (a) Thirty (30) days’ advance written notice shall be provided to the City of cancellation, intended non-renewal, or reduction in coverages, except for non-payment for which no less than ten (10) days’ notice shall be provided to City. Notices shall be sent to the City address set forth in Section 11.1 entitled “Notices to the Parties.”

  • Insurance Endorsements The insurance policies shall contain the following provisions, or Consultant shall provide endorsements on forms supplied or approved by the City to add the following provisions to the insurance policies:
