Certification Submittals. Completion records for all classroom courses, seminars, and self-study courses are due to the Provider from the Sponsor no more than 15 calendar days after the course completion date. All completion records must include the operator’s name, license type, license number, address, course or seminar number, course name/title, completion date, and number of CEUs earned. Completion records shall then be uploaded/transmitted by the Provider to the Department in an electronic format approved by the Department no more than 30 calendar days after course completion date. However, the continuing education provider shall electronically report to the Department completion of a licensee’s course within 10 calendar days beginning on the 30th day before the renewal deadline or prior to the renewal date, whichever occurs sooner. The Provider shall be deemed in compliance if their timely upload percentage rate at the end of the two-year license cycle is 95% or higher.

  • AS9100 Certification ‌ AS9100 Certification, specifies requirements for a quality management system to demonstrate the Contractor’s ability to consistently meet the customer requirements as well as statutory and regulatory requirements for the aerospace industry. An AS9100 Certification, is not mandatory; however, Contractors who desire to compete for work within the aerospace industry are encouraged to have AS9100 Certification, during the entire term of OASIS. The Contractor shall notify the OASIS CO, in writing, if there are any changes in the status of their AS9100 Certification, and provide the reasons for the change and copies of audits from an AS9100 Certification Body, as applicable. If only part of a Contractor’s organization is AS9100, certified, the Contractor shall make the distinction between which business units or sites and geographic locations have been certified.

  • Required Certifications A bidder who makes a false certification on the Bidder Certification of the Prequalification Classification and Work Capacity Form will be subject to forfeiture of his bid bond and/or disqualification from bidding on future work for a 90 day period, or both. The Contracting Officer will determine the imposition and the extent of such sanctions. A sworn statement shall be executed by the bidder or his agent on behalf of each person, firm, association, or corporation submitting a bid. The statement shall certify that the person, firm, association, or corporation has not, either directly or indirectly, entered into any agreement, participated in any collusion, or otherwise taken any action to restrain free competitive bidding in connection with the proposal. The sworn statement shall be part of the bid or in the form of an affidavit and shall be sworn to before a person who is authorized by the laws of the State to administer oaths. The bids shall contain the identical sworn statement. For the purpose of this Section, affixing digital ID to the bid will be considered by the Department conditional evidence of signing before a person who is authorized by the laws of the Commonwealth to administer oaths.   • Instructions for Certification 1. By signing and submitting this CONTRACT, the prospective lower tier participant is providing the certification set out below.

  • Certification of Contractor The Contractor shall certify, over his own signature, that the Work provided for by the Contract Documents has been completed under the terms and conditions thereof, and that the entire balance of the contract is due and payable.

  • Certification of Completion Certification of completion of the program will be contingent upon Resident successfully completing the core requirements of the Program, returning all property of the Medical Center such as books, equipment, etc., and having settled any other professional or financial obligations to the Medical Center. The certificate will be issued in accordance with the provisions of the ASHP Regulations on Accreditation of Pharmacy Residencies and signed by the RPD and the Chief Executive Officer of the organization. A certificate will not be issued to anyone who does not complete the program’s requirements. Core requirements to successfully complete the program include: • Possess a valid pharmacist license from the Pennsylvania State Board of Pharmacy by October 31st of the residency year. • Successful completion of core learning experiences and required longitudinal learning experiences as defined in the learning experience descriptions • Completion of all R1 goals/objectives with a status of achieved by the end of the residency. Completion of 100% of the remaining goals/objectives for the program with at least 50% of the objectives having a status as achieved. All objectives must have a minimum status of satisfactory progress. • All evaluations must be completed in accordance with ASHP Accreditation Standard as outlined in the Resident Agreement • Completion of pharmacokinetic certification by October 31 of residency year • Completion of inpatient anticoagulation certification by October 31 of residency year. • Completion of major project, which includes meeting all project deadlines and submission of written manuscript of completed project to the RAC. • Assigned projects (MUE, drug review, class review, and management project) have been completed • Presentation of at least one major CME program. • Completion of minimum staffing requirements as described in section 2.7 • Adherence to the organization’s attendance standards and as described in this document under Exhibit A.

  • LEED Certification Landlord may, in Landlord’s sole and absolute discretion, elect to apply to obtain or maintain a LEED certification for the Project (or portion thereof), or other applicable certification in connection with Landlord’s sustainability practices for the Project (as such sustainability practices are to be determined by Landlord, in its sole and absolute discretion, from time to time). In the event that Landlord elects to pursue such an aforementioned certification, Tenant shall, at Tenant’s sole cost and expense, promptly cooperate with the Landlord’s efforts in connection therewith and provide Landlord with any documentation it may need in order to obtain or maintain the aforementioned certification (which cooperation may include, but shall not be limited to, Tenant complying with certain standards pertaining to the purchase of materials used in connection with any Alterations or improvements undertaken by the Tenant in the Project, the sharing of documentation pertaining to any Alterations or improvements undertaken by Tenant in the Project with Landlord, and the sharing of Tenant’s billing information pertaining to trash removal and recycling related to Tenant’s operations in the Project).

  • ISO 9001 Certification The Contractor shall maintain or exceed their ISO 9001 Certification and submit updates, if applicable

  • Recertification Upon expiration of the time period which the health care provider originally estimated that the employee needed for his/her own serious health condition, the City may require the employee to obtain recertification if additional leave is requested.
