Certification of Completion. Upon completion of administration of the Settlement, the Settlement Administrator will provide a written declaration under oath to certify such completion to the Court and counsel for all Parties.
Certification of Completion. Certification of completion of the program will be contingent upon Resident successfully completing the core requirements of the Program, returning all property of the Medical Center such as books, equipment, etc., and having settled any other professional or financial obligations to the Medical Center. The certificate will be issued in accordance with the provisions of the ASHP Regulations on Accreditation of Pharmacy Residencies and signed by the RPD and the Chief Executive Officer of the organization. A certificate will not be issued to anyone who does not complete the program’s requirements. Core requirements to successfully complete the program include: • Possess a valid pharmacist license from the Pennsylvania State Board of Pharmacy by October 31st of the residency year. • Successful completion of core learning experiences and required longitudinal learning experiences as defined in the learning experience descriptions • Completion of all R1 goals/objectives with a status of achieved by the end of the residency. Completion of 100% of the remaining goals/objectives for the program with at least 50% of the objectives having a status as achieved. All objectives must have a minimum status of satisfactory progress. • All evaluations must be completed in accordance with ASHP Accreditation Standard as outlined in the Resident Agreement • Completion of pharmacokinetic certification by October 31 of residency year • Completion of inpatient anticoagulation certification by October 31 of residency year. • Completion of major project, which includes meeting all project deadlines and submission of written manuscript of completed project to the RAC. • Assigned projects (MUE, drug review, class review, and management project) have been completed • Presentation of at least one major CME program. • Completion of minimum staffing requirements as described in section 2.7 • Adherence to the organization’s attendance standards and as described in this document under Exhibit A.
Certification of Completion. Certification of completion of the program will be contingent upon, including but not limited to, Resident returning all property of the Medical Center such as books, equipment, or protected health information, and having completed all medical records for which he or she is responsible, as well as, having settled any other professional or financial obligations to the Medical Center.
Certification of Completion. No later than the Compliance Project completion dates identified in Attachment B, a responsible official of the Discharger shall submit a final report and certified statement, signed under penalty of perjury, which documents that the Discharger completed the Compliance Project in accordance with the terms of this Stipulated Order, and documents the Discharger’s expenditures to implement the Compliance Project. Documentation may include photographs, invoices, receipts, certifications, and other materials reasonably necessary for the Central Valley Water Board to evaluate completion of the Compliance Project and the costs incurred. The Discharger shall provide Central Valley Water Board staff with any additional information that is reasonably necessary to verify the Discharger’s expenditures and certification of completion.
Certification of Completion. Only upon satisfactory completion of the Residency Program and the final clearance procedure of the Hospital, shall the Hospital furnish to the Resident a certificate of completion. The final clearance process consists of but may not be limited to the return of all hospital property such as scrubs, laboratory coats and hospital identification cards. The clearance process shall also include the completion of outstanding patient medical charts.
Certification of Completion. Within 30 days of completion of the ECA, the City shall submit a certified statement of completion of the ECA (“Certification of Completion”). The City’s authorized representative shall submit the Certification of Completion under penalty of perjury to the designated Central Valley Water Board contact. The Certification of Completion shall include the following:

OF APPRENTICESHIP Upon completion of the apprenticeship under these apprenticeship standards, the Joint Apprenticeship Committee will recommend to the Industrial Training Branch, Ministry of Skills Development, that a certificate signifying completion of the apprenticeship be issued to the apprentice. No certificates will be issued by the Industrial Training Branch, Ministry of Skills Development, unless approved by the Joint Apprenticeship Committee.
Certification of Completion. The interconnection Customer shall provide the Utility with a copy of the Certificate of Completion with all relevant and necessary information fully completed by the interconnection Customer, as well as an inspection form from the local electrical inspection authority demonstrating that the generating facility passed inspection.
Certification of Completion. Upon fully administering this Agreement, the 27 Settlement Administrator will certify the completion to the Court and counsel for all Parties in a 28 declaration, summarizing the total money paid and the status of any uncashed checks.
Certification of Completion. Upon satisfactory completion of the Residency Program, SUNY Downstate Medical Center will furnish to the resident a Certificate of Completion. Physicians can focus on a specific area of a specialty and earn subspecialty certification that is essential for careers in teaching, research or practice restricted to that field. Specific information related to the eligibility for specialty board exams will be provided by the training program and can be accessed at the specialty board websites. See Certification in Section II of the Resident Handbook.
