Change of Details Form. 8.2.1 You must properly complete and give us a Change of Details Form and give us the Supporting Documents that we may ask for. The Supporting Documents we need will depend on the details that you are changing. 8.2.2 You can get this Change of Details Form and the list of Supporting Documents that we will need, from the Transaction Platform Website or you can phone the Call Centre and we will send it to you. 8.2.3 After we have received and checked the Change of Details Form and the Supporting Documents we have asked for, we will send confirmation by SMS to you or, in the case of a Black Group, to your Authorised Representative.
