COLLEGE RESPONSIBILITIES. a. m. n. The College shall ensure that all Interns are aware of and have complied with the Facility’s policies regarding the need for any health screenings, vaccinations, criminal background checks and/or other Facility requirements prior to the commencement of the Fieldwork.

The College shall provide the Facility with student information, medical/immunization records, professional liability insurance information, health insurance information (including names of insurance companies and policy numbers), and any other forms required by the Facility, at least one month prior to the start of the affiliation.
o. p. The College will ensure that each Intern will sign a notice, in a form attached hereto as Exhibit A, prior to starting their internship, and will provide a signed copy of each Intern’s notice to the Facility.
q. The College will inform all Interns that they are responsible for:
i. following the Facility’s rules, regulations, policies and procedures;
ii. paying all applicable expenses, including without limitation meals, laundry of uniforms, medical expenses, transportation and books; and

COLLEGE RESPONSIBILITIES. a. The College shall: Assume and maintain sole and full responsibility for the planning, development, and execution of the educational component of the OTA and OT program including administration, faculty appointments, curriculum planning, development and revision, and the requirements for matriculation, promotion and graduation. Use its best efforts in referring to the Facility only those students who meet such academic and other qualifications as are consistent with the objectives and requirements of the Fieldwork Training. Only students who have completed all program coursework in its entirety will place be considered for a Level II Fieldwork Training position. Provide participants with education regarding bloodborne pathogens appropriate to the participant’s educational training at the Facility, and shall maintain documentation of such education. Provide each student Facility information to assist the student in his/her placement in the Fieldwork only those Interns who are registered in The Sage CollegesTraining. Use its best efforts to ensure that faculty and students comply with all rules, Program Dietetic Internship , regulations and who have successfully completed any appropriate prerequisite courses and/or other non-clinical educational experience.
b. The College will inform procedures of the Facility of the level of training each student has received and will provide current information regarding during the student’s non-clinical educational goals.
c. The College shall determine the expectations Fieldwork Training. Provide qualified faculty, who are appropriately licensed or certified, and requirements for Fieldwork participation.
d. The College shall provide the Facility Representative with copies of applicable curricula.
e. The College shall notify the Facility of any changes in the academic curriculum.
f. The College shall provide time and opportunity for orientation of Facility personnel to the Dietetic Internship Program of the College.
g. The College shall notify the Facility when there is a change in the staffing of Academic Fieldwork Coordinator.
h. The College shall provide all coordination and assignment of Interns make such information available to the Facility upon request. Faculty shall be responsible for practical experiences.
i. The College shall contact fieldwork coordination and for coordination of the supervising non-clinical instructor student Fieldwork Training with the Facility staff, including the planning and selecting of Fieldwork Training. Provide and maintain during the affiliation term of the agreement, commercial general liability insurance in the amount of $1,000,000 each occurrence; $3,000,000 Policy Aggregate. Require that each student, prior to discuss student performance.
j. The College shall have the right being permitted to remove an Intern from the commerce a Fieldwork at any timeTraining, in accordance with the College’s policies.
k. The College shall ensure that all Interns obtain and maintain health insurance covering accidents and sickness with such companies and with such terms and limits as provide proof to the College shall reasonably require.
l. The College shall ensure that all Interns obtain and maintain personal the student is covered by policy of professional liability insurance in the amount of no less than $1,000,000/ 1,000,000 each occurrence and $3,000,000 Policy Aggregate. Said policies shall remain in full force and effect during the term of such student’s clinical experience. Withdraw or transfer any faculty or students from the Fieldwork Training due to reasons of health, unsatisfactory performance of required duties, failure to maintain client and/or patient safety, failure to comply with such companies relevant College or Facility policies and with such terms and limits as procedures, or inability to continue to productively participate in the College shall reasonably require.
m. Fieldwork Training. The College shall ensure that all Interns are aware of and have complied with reason(s) for the Facility’s policies regarding request to withdraw the need for student from Fieldwork Training will be provided to the College in writing and, whenever possible, the student will be afforded the opportunity to respond in writing to the College. Notwithstanding the foregoing, in any health screeningsinstance where Facility supervisory personnel deem any student a potential threat to the quality of care given to client and/or patient(s) the student will be withdrawn or transferred immediately, vaccinationsand the written explanation(s) will be supplied by the Facility as promptly thereafter as is practicable. Use its best efforts to provide the Facility with information concerning the number of students to be assigned to the Facility and dates of participation, criminal background checks and/or other prior to the subject to approval by the Facility requirements prior to the commencement of the Fieldwork.
n. The relevant Fieldwork Training period. Instruct all students and faculty involved in the Fieldwork Training to follow Facility rules and procedures pertaining to confidential information relating to persons receiving care in the Facility. Students and College personnel shall provide not be permitted pictures; slides or their identifiable reproductions of any client and/or patient and the Facility, subject to such conditions as may be required. Provide the Facility with student information, medical/immunization records, professional liability insurance information, health insurance information current evidence of student: Rubella Screen (including names copy of insurance companies and policy numbersPositive Antibody Test) Measles Vaccine (for students born on or after January 1, 1957), proof of two doses of prior vaccine or documentation of disease and copy of Antibody Test Measles/Mumps/Rubella Vaccine (dates of vaccine or titer results) Mantoux (PPD) Test (2-step mantoux performed within the last 12 months) Hepatits B (dates of vaccine and titer results) Chicken pox (evidence of immunity or laboratory confirmation of disease) Diphtheria (primary series and booster within last 2 years) Tetanus (primary series and booster within last 2 years) Polio (primary series and booster) Tetanus, Diphtheria, and Pertussis (Tdap) (documentation of vaccine within 2 years) Physical examinations (performed within the last 6 months) CPR Certification BCI Documentation Provide Worker’s Compensation Insurance coverage for its participating faculty members but not students. MUTUAL RESPONSIBILITIES The College and Facility shall: Each appoint a coordinator to oversee the implementation of the Fieldwork Training. The coordinators shall confer at regular intervals to insure maintenance of the high educational level of a student’s Fieldwork Training, and shall be available at any time regarding any aspect of a student’s Fieldwork Training. Through their respective coordinators, jointly plan the utilization of clinical facilities, and the development and coordination of student activities related to the Fieldwork Training. Work together to maintain an environment of quality learning experience for the College’s students while at the same time, enhancing the resources available to the Facility for providing care to its clients and/or patients. Require participants to be identified as students by dress, badge or other appropriate means. It is understood that students are not employees of the Facility for any purposes under this agreement. Not withstanding any other forms required by provisions of this agreement, the Facility, Facility and the College agree that each student participating the Fieldwork Training shall at least one month prior to the start all times remain students of the affiliation.
o. The College will inform all Interns that they are responsible for:
i. following the Facility’s rules, regulations, policies and procedures;
iiCollege. paying Comply in every respect with all applicable expensesprovisions of all City, including without limitation mealsState and Federal statutes, laundry rules and regulations which prohibit unlawful discrimination against any employee, applicant for employment, student or applicant for admission because of uniformsrace, medical expensescolor, transportation religion, sex, disability, age, or national and books; and
iii. providing the Facility with records of the Intern’s immunizations and/or other medical tests as may be required by the Facilityethnic origin.

COLLEGE RESPONSIBILITIES. a. The College will place in the Fieldwork only those Interns who are registered in The Sage Colleges, Program of Dietetic Internship , and who have successfully completed any appropriate prerequisite courses and/or other non-clinical educational experience.
b. The College will select and assign the Interns, who shall not be subject to any employment recruitment process or screening by the Facility.
c. The College will inform the Facility of the level of training each student has received and will provide current information regarding the student’s non-clinical educational goals.
c. d. The College shall determine the expectations and requirements for Fieldwork participation, and award academic credit for the Intern’s successful completion of the Fieldwork.
d. e. The College shall provide the Facility Representative with copies of applicable curricula.
e. f. The College shall notify the Facility of any changes in the academic curriculum.
f. g. The College shall provide time and opportunity for orientation of Facility personnel to the Dietetic Internship Program of the College.
g. h. The College shall notify the Facility when there is a change in the staffing of Academic Fieldwork Coordinator.
h. i. The College shall provide all coordination and College’s assignment of Interns to the Facility for practical experiencesexperiences shall be for a pre-determined period of time, which shall not be extended.
i. j. The College shall contact the supervising non-clinical instructor during the affiliation to discuss student performance.
j. k. The College shall have the right to remove an Intern from the Fieldwork at any time, in accordance with the College’s policies.
k. l. The College shall ensure that all Interns obtain and maintain health insurance covering accidents and sickness with such companies and with such terms and limits as the College shall reasonably require.
l. m. The College shall ensure that all Interns obtain and maintain personal professional liability insurance in the amount of $1,000,000/ $3,000,000 with such companies and with such terms and limits as the College shall reasonably require.
m. n. The College shall ensure that all Interns are aware of and have complied with the Facility’s policies regarding the need for any health screenings, vaccinations, criminal background checks and/or other Facility requirements prior to the commencement of the Fieldwork.
n. o. The College shall provide the Facility with student information, medical/immunization records, professional liability insurance information, health insurance information (including names of insurance companies and policy numbers), and any other forms required by the Facility, at least one month prior to the start of the affiliation.
o. p. The College will inform all Interns that they are responsible for:
i. following the Facility’s rules, regulations, policies and procedures;
ii. paying all applicable expenses, including without limitation meals, laundry of uniforms, medical expenses, transportation and books; and
iii. providing the Facility with records of the Intern’s immunizations and/or other medical tests as may be required by the Facility.
