Common use of Comprehensive Care Planning Clause in Contracts

Comprehensive Care Planning. Physician will continually work with Program Member to set Program Member’s health goals, create a plan to achieve those goals and to monitor progress in achieving those goals. Physician will also provide health coaching to address obstacles to health improvement.
