Confidentiality of Client Information. a. All information as to personal facts and circumstances obtained by the Provider on the Participant shall be treated as privileged communications, shall be held confidential, and shall not be divulged without the written consent of the Participant his or her guardian, or the responsible parent when the Participant is a minor child, or except as required by other terms of this Agreement. Provider shall treat every aspect of services under this Agreement as confidential, including the fact of eligibility and/or enrollment and any or all information pertaining to a Participant’s physical or mental health status or condition.
