Contact Tracing. Residents who test positive for an infectious disease or come into close contact with someone who tests positive for an infectious disease are required to complete a Close Contact or Personal Diagnosis Form and are strongly encouraged to cooperate with public health officials’ quarantine orders and contact tracing efforts.
