Content of Summary. The content of the summary of Disclosure(s) shall contain the following information: (i) date of the Disclosure(s); (ii) name of the entity or person who received the EHI and, if known, the address of such entity or person; (iii) brief description of the EHI disclosed; and (iv) brief statement of the purpose of the Disclosure(s) that reasonably informs the Individual of the basis for the Disclosure(s) or, in lieu of such statement, a copy of the written request for the Disclosure(s).
