DESCRIPTION OF CONTRACT MODIFICATION. This contract modification is made in accordance with Exhibit E-Revised-1, Contractual Terms and Conditions, Section 22. CHANGES, to be made part hereof for all pertinent purposes. The changes are as follows:
DESCRIPTION OF CONTRACT MODIFICATION. This modification is issued in accordance with Exhibit E-Revised - 3, Section 19. Changes, as a result of recent legislative updates to federally mandated clauses which apply to all federally assisted supply and service contracts.
1. Refer to Exhibit B, Representations and Certifications (Federally Assisted Supply/Service/Construction Contracts). Exhibit B, is deleted in its entirety and replaced with Exhibit B-Revised-1, Representations and Certifications (Federally Assisted Supply/Service/Construction Contracts), attached hereto and made a part hereof for all pertinent purposes. Please complete, sign, and return this form along with the signed Contract Modification form.
2. Refer to Exhibit E–1, Addendum to Contractual Terms and Conditions, Federally Assisted Supply or Service Contract. Exhibit E-1 is deleted in its entirety and replaced with Exhibit-E-1-Revised-1, Addendum to Contractual Terms and Conditions, Federally Assisted Supply or Service Contract, attached hereto and made a part hereof for all pertinent purposes.
DESCRIPTION OF CONTRACT MODIFICATION. This Modification 1 is issued to exercise Option 1 of the Contract (Year 2) in accordance with Exhibit E-Revised-1, Contractual Terms and Conditions, Section 4, Option to Extend Term for the Contract prices in Exhibit A-Revised- 2, Pricing Schedule, Section 8 thru 8H inclusive. The Term of the Contract is hereby extended to May 1, 2023.
DESCRIPTION OF CONTRACT MODIFICATION. This Modification is in accordance with Exhibit E-Revised, Contractual Terms and Conditions, Section 4, Option to Extend Term, to be made a part hereof for all pertinent purposes. Modification 3 exercises option year 3 of the contract. Pricing for option year 3 is based on fully burdened labor rates for task orders as indicated in Exhibit A-1, Section 4. The new Contract expiration date is September 1, 2021.
DESCRIPTION OF CONTRACT MODIFICATION. This modification makes the following changes to the Contract for all pertinent purposes:
1. Extend the term of the Contract for 12 months, for pricing in Exhibit A-1 , Pricing, Fully Burdened Labor Rates - Option 1, Year 2, in accordance with Exhibit E Revised-3, Contractual Terms and Conditions, Section 4, Option To Extend Term. The new Contract expiration date is February 23, 2022.
DESCRIPTION OF CONTRACT MODIFICATION. This modification makes the following change to the Contract for all pertinent purposes:
DESCRIPTION OF CONTRACT MODIFICATION. This Modification 3 is issued in accordance with Exhibit E, Contractual Terms and Conditions, Section 4, OPTION TO EXTEND CONTRACT TERM to be made a part hereof for all pertinent purposes. The changes are as follows:
1. This Modification 3 is issued to exercise a twelve (12) month unilateral extension option in accordance with Exhibit E, Contractual Terms and Conditions, Section 4, entitled OPTION TO EXTEND CONTRACT TERM for the fully burdened labor rates specified in Exhibit A-1-Revised-1, Pricing Schedule, Section 8.D., Option Year 3 (Contract Year 4).
2. The Term of the Contract is hereby extended to October 25, 2023.
DESCRIPTION OF CONTRACT MODIFICATION. This modification makes the following change to the Contract for all pertinent purposes: Refer to Exhibit E – Revised-1, Contractual Terms and Conditions. Exhibit E – Revised-1 shall be replaced in its entirety with Exhibit E- Revised-2, attached hereto and made a part hereof for all pertinent purposes. Additional language has been made to Section 33, Request for Information. Changes are reflected with highlighted text and a line in the margin.
(d) The requirements of Subchapter J, Chapter 552, Government Code, may apply to this contract and the contractor or vendor agrees that the contract can be terminated if the contractor or vendor knowingly or intentionally fails to comply with a requirement of that subchapter.
1. The requirement of Subchapter J, Chapter 552, Government Code as amended currently applies to expenditures of at least $1 million in public funds for the purchase of goods or services.
DESCRIPTION OF CONTRACT MODIFICATION. This modification makes the following changes to the Contract for all pertinent purposes in accordance with Exhibit E Revised-1 Contractual Terms and Conditions, Section 19, Changes:
1. This Modification adds $250,000 to the contract amount for Task Order 7.1 revision in the amount $245,963.04. No additional time is added to the contract.
DESCRIPTION OF CONTRACT MODIFICATION. This modification makes the following changes to the Contract for all pertinent purposes:
1. Extend the term of the Contract for 12 months, for pricing in Exhibit A-1, Section 7E, Pricing, Option Year 4 (Year 5 of the Contract), in accordance with Exhibit E revised-1, Contractual Terms and Conditions, Section 4, Option to Extend Term. The new Contract expiration date is October 30, 2022.
