DESCRIPTION OF CONTRACT MODIFICATION. This modification is issued in accordance with Exhibit E-Revised - 3, Section 19. Changes, as a result of recent legislative updates to federally mandated clauses which apply to all federally assisted supply and service contracts. 1. Refer to Exhibit B, Representations and Certifications (Federally Assisted Supply/Service/Construction Contracts). Exhibit B, is deleted in its entirety and replaced with Exhibit B-Revised-1, Representations and Certifications (Federally Assisted Supply/Service/Construction Contracts), attached hereto and made a part hereof for all pertinent purposes. Please complete, sign, and return this form along with the signed Contract Modification form. 2. Refer to Exhibit E–1, Addendum to Contractual Terms and Conditions, Federally Assisted Supply or Service Contract. Exhibit E-1 is deleted in its entirety and replaced with Exhibit-E-1-Revised-1, Addendum to Contractual Terms and Conditions, Federally Assisted Supply or Service Contract, attached hereto and made a part hereof for all pertinent purposes.
DESCRIPTION OF CONTRACT MODIFICATION. This Modification 1 is issued to exercise Option 1 of the Contract (Year 2) in accordance with Exhibit E-Revised-1, Contractual Terms and Conditions, Section 4, Option to Extend Term for the Contract prices in Exhibit A-Revised- 2, Pricing Schedule, Section 8 thru 8H inclusive. The Term of the Contract is hereby extended to May 1, 2023.

The requirement of Subchapter J, Chapter 552, Government Code as amended currently applies to expenditures of at least $1 million in public funds for the purchase of goods or services.
