Common use of Continuation of Benefits to Survivors Clause in Contracts

Continuation of Benefits to Survivors. The District shall provide hospitalization and major medical coverage for the surviving dependents of its employees who possess family coverage under the District's hospitalization policy for a minimum period established by law following the covered employee's death. Coverage shall not be terminated except upon the written consent of the survivor or survivors of any deceased covered employee or termination of the plan or non-payment of premiums. However, any survivor or survivors, in order to have the coverage and benefits extended for such period, as herein provided, shall be required to pay the entire cost of such protection in monthly installments. Failure of the survivor to make premium payments in advance after written notice from the employer has been received, shall be a basis in itself for the termination of the coverage, provided the School Board has notified the survivor in writing fifteen (15) days before notification of pending cancellation.
