Common use of COVID-19 Vaccination / Testing Requirements Clause in Contracts

COVID-19 Vaccination / Testing Requirements. Vaccination Requirements Architect shall fill out, sign, date and submit to District the COVID-19 Vaccination/Testing Certification Form, attached hereto. According to the August 11, 2021, California Department of Public Health (“CDPH”) State Public Health Officer Order (“Order”), a person is “fully vaccinated” for COVID-19 if two weeks or more have passed since they have received the second dose in a 2-dose series (Pfizer-BioNTech or Moderna or vaccine authorized by the World Health Organization), or two weeks or more have passed since they received a single-dose vaccine (Xxxxxxx and Xxxxxxx[J&J]/Xxxxxxx). Pursuant to the CDPH Guidance for Vaccine Records Guidelines & Standards, Architect shall only accept the following as proof of vaccination:
