Credentialing Requirements. Practitioner shall comply fully with Health Plan credentialing requirements applicable to Practitioner and health care personnel supervised by Practitioner. Practitioner shall promptly notify Health Plan and Provider of any material changes in professional licensure, hospital privileges or other professional status. To the extent Health Plan delegates credentialing to Practitioner, Health Plan shall review and approve the credentialing process and will audit the credentialing process on an ongoing basis.
