Data Case Report Forms. Prior to submission of the paper CRF, all forms should be filled out completely. Data from the CRF must be either verifiable against source documents, or have been directly entered into the CRF, in which case the entry in the CRF will be considered as the source data. All data will be entered in the study database and validated. Missing or implausible data will be queried. The investigator is responsible for the accuracy and completeness of data reported on the CRFs. Each set of patient CRFs must be reviewed and signed by the investigator.
