DOCTOR'S CERTIFICATE - FIFTH DAY. a) Any employee whose illness extends to five (5) working days shall, on or before the fifth (5th) day, file a Doctor’s certificate with the Department Head and/or supervisor. b) Notwithstanding the foregoing, the Employer may require an employee to provide a Doctor’s certificate and/or a Fitness to Work Form for any absences of less than five (5) days where there is a demonstrated pattern of absences.
