Documentation of Uses and Disclosures. The Business Associate shall document such disclosures of PHI and information related to such disclosures as would be required for the Contractor to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with 45 Code of Federal Regulations section 164.528.

Documentation of Uses and Disclosures. Business Associate shall document all disclosures of PHI/EPHI and information related to such disclosures in a manner as would be required for the County health care component to respond to a request by an individual for an accounting of disclosures of PHI/EPHI in accordance with the Privacy Rule and 45 CFR 164.528.
Documentation of Uses and Disclosures. BUSINESS ASSOCIATE shall document any disclosures of PHI in a manner that would allow the COUNTY to respond to a request by an Individual for an “accounting of disclosures of PHIin accordance with Title 45, CFR, Section 164.528.
