Accounting of Disclosures of PHI a. Vendor shall document such disclosures of PHI and information related to such disclosures as would be required for the Board to respond to a request by an individual for an accounting of disclosures of PHI in accordance with Title 45, Part 164, Subpart E, Section 164.528 of the CFR. b. Vendor agrees to provide the Board or an individual, in the time and manner set forth in the Board's health information privacy and security policies and procedures, information collected in accordance. with Section 10 (i) above, to permit the Board to respond to a request by an individual for an accounting of disclosures of PHI in accordance with Title 45, Part 164, Subpart E, Section 164.528 of the CPR.

Accounting of Disclosures of PHI. Business Associate must: Maintain and make available the information required to enable Covered Entity to respond to an individual’s request for an accounting of disclosures of Protected Health Information in accordance with 45 C.F.R. § 164.528; and Within five (5) days of receiving a written request from Covered Entity, provide to Covered Entity such information as is requested by Covered Entity to permit Covered Entity to respond to a request by an individual for an accounting of the disclosures of the individual's Protected Health Information in accordance with 45 C.F.R. § 164.528. In the event that an individual requests an accounting of disclosures directly from Business Associate or its agents or Subcontractors, Business Associate must notify Covered Entity in writing within five (5) days of the request; Covered Entity will be responsible for preparing and delivering to the individual any such accounting requested. Make available all records, books, agreements, policies and procedures relating to the use or disclosure of Protected Health Information to the Secretary of the U.S. Department of Health and Human Services (or any officer or employee of HHS to whom the Secretary of HHS has delegated such authority) for purposes of determining Covered Entity’s compliance with the Privacy Rule and Security Rule, subject to attorney-client and other applicable legal privileges. Business Associate will immediately notify Covered Entity upon receipt by Business Associate of any complaint or request for access by the Secretary of HHS and will provide Covered Entity with a copy thereof as well as a copy of all materials disclosed pursuant thereto. Upon reasonable prior written notice, make available during normal business hours at Business Associate’s offices all policies, procedures and related materials pertaining to the use or disclosure of, and security of, Protected Health Information to Covered Entity for purposes of enabling Covered Entity to determine Business Associate’s compliance with the terms of this Agreement.
Accounting of Disclosures of PHI. BCBSRI shall maintain a record of Disclosures of the Plan’s PHI and provide the record to Employer as necessary for the Plan’s compliance with 45 C.F.R. § 164.528.
Accounting of Disclosures of PHI. Business Associate shall make available to Covered Entity such information as Covered Entity may require to fulfill Covered Entity’s obligations to provide an accounting of disclosures with respect to PHI in accordance with HIPAA Rules. Accounting of Disclosures of PHI. BA shall make available to CE such information as CE may require to fulfill CE’s obligations to provide an accounting of disclosures with respect to PHI in accordance with HIPAA. In addition, BA shall maintain a record of all disclosures of PHI, including the date of the disclosure, the name and, if known, the address of the recipient of the PHI, a brief description of the PHI disclosed, and the purpose of the disclosure which includes an explanation of the basis for such disclosure. BA shall make this record available to CE upon CE’s request.
Accounting of Disclosures of PHI. Within five (5) business days of receipt of a request from Covered Entity, Business Associate shall make available to Covered Entity a list of disclosures of PHI as required for Covered Entity to fulfill its obligations to provide an accounting pursuant to the Privacy Rule, including but not limited to, 45 CFR § 164.528. Business Associate shall implement a process that allows for such an accounting. If an Individual requests such an accounting directly from Business Associate, Business Associate must notify Covered Entity in writing within five (5) business days.

Accounting of Disclosures of PHI. Business Associate, and any Subcontractor acting on its behalf, must account for all Disclosures of PHI for which a Covered Entity must account for to comply with 45 C.F.R. § 164.528, as may be amended. Upon the request of Sutter, Business Associate shall provide to Sutter within five (5) business days an accounting of all Disclosures of PHI in order for Sutter to comply with 45 C.F.R. § 164.528, as may be amended. Business Associate shall provide the information required under 45 C.F.R. § 164.528(b), including, as applicable, the date of the Disclosure, the name and, if known, the address of the recipient of the PHI, a brief description of the PHI Disclosed, and the purpose of the Disclosure. If an Individual contacts Business Associate directly for such an accounting, Business Associate shall direct the Individual to contact Sutter.
