Common use of Empanelment for Cancer Clause in Contracts

Empanelment for Cancer. Provider agrees to provide the services of fully qualified surgical oncologist and medical oncologist to treat beneficiaries requiring surgical and chemotherapy treatments. Further to treat beneficiaries requiring surgical, chemotherapy and radiotherapy treatments, it shall provide the services of surgical oncologist, medical oncologist, radiation oncologist and equipment for Cobalt therapy, Linear accelerator, 3DCRT, IMRT, SRT/SRS and Brachytherapy for empanelment under Rajiv Arogya Bhagya. In case facilities such as Chemotherapy and Radiotherapy are not available in the treating hospital, it shall coordinate with the other tied-up co-Provider for such necessitated procedures who are well-equipped with it. In furtherance to this, the main provider shall get into a contractual tie-up which such co-providers and pay for it. The Provider agrees to follow to constitute a “Tumor Board” as directed by XXXX for the diagnosis and treatment of the Cancer Patients. Further, the Provider also agrees to follow the Standard Treatment Guidelines published by the Trust for the treatment of Cancer Patients. Chemotherapy, Nuclear medicine and Radiotherapy should be administered only by empanelled professionals well versed in dealing with it. Beneficiaries with Hematological malignancies (Ex. Leukemia, Lymphomas and Multiple Myeloma) and Pediatric malignancies (Any patient < 14 years of age) should be treated by empanelled medical oncologist only. Chemotherapy shall be administered to the patients, treating them as “in-patient/daycare.” Radiotherapy shall be administered to the patients, treating them as “in-patient/daycare". To the “daycare patients” the Hospital shall provide accommodation with medical supervision. Provider agrees to provide the services as per the Benefit Packages and adhere to the treatment protocols, referred to at ANNEXURE-VIII. Provider agrees to quote batch no. of the drugs and attach photographs of empty vials and ampoules with labels intact along with the bills for those consumables whose MRP is more than Rs. One thousand only(Rupees 1000/-) Provider shall agree to give patient feedback through multimedia using webcam and xxxx. The provision for live viewing of the patient would be provided to the Trust portal.

(Annexure – VI) ⮚ Provider agrees to provide the services form tumour board consisting of fully qualified surgical oncologist Surgical Oncologist, Radiologist and medical Medical oncologist to treat beneficiaries requiring surgical surgical, Radio therapy and chemotherapy treatments. Further Further, to treat treating (Tumour board format - Annexure- VII) beneficiaries requiring surgical, chemotherapy and radiotherapy treatments, it the Provider shall provide the services of surgical oncologist, medical oncologist, radiation oncologist and equipment for Cobalt therapy, Linear accelerator, accelerator radiation treatment 3DCRT, /IMRT, /SRT/SRS and Brachytherapy for empanelment under Rajiv Arogya BhagyaSAST Schemes. In case facilities such as Chemotherapy and Radiotherapy are not available in the treating hospital, it they shall coordinate with the other tied-up co-Provider for such necessitated procedures who are well-equipped with it. In furtherance to this, the main provider shall get into have a contractual tie-up which such co-providers and pay for itrespective specialities to provide multi- disciplinary treatment. The Provider agrees to follow to constitute a “Tumor Board” as directed by XXXX for the diagnosis and treatment of the Cancer Patients. Further, the Provider also agrees to follow the Standard Treatment Guidelines published by the Trust for the treatment of Cancer Patients. Chemotherapy, Nuclear medicine (Oncology tie up annexure -VIII) ⮚ Chemotherapy and Radiotherapy should be administered only by empanelled professionals well versed in dealing with itthe side-effects. Beneficiaries with Hematological malignancies (Ex. Eg : Leukemia, Lymphomas and Multiple Myeloma) and Pediatric Paediatric malignancies (Any patient < 14 beneficiaries<14 years of age) should be treated by empanelled medical oncologist qualified Medical oncologist/Paediatric Oncologist only. Chemotherapy / RT can be administered to the patients as in-patients/day care treatment. The package rate will be as per general xxxx rates. ⮚ In case of providing day care, the provider shall ensure hospital/dormitory accommodation with medical supervision. ⮚ Radiotherapy can be administered to the patients, treating them as “in-in – patient/daycare.” Radiotherapy shall be administered to the day care patients, treating them as “in-patient/daycare". To the “daycare day care patients” the Hospital hospital shall provide accommodation with treatment under medical supervision. For patients coming from far, RT can be provided as in patient. ⮚ Provider agrees to provide the services as per the Benefit Packages packages and adhere to the treatment protocols, referred to at ANNEXURE-VIII. Provider agrees to quote batch no. number of the Chemotherapy drugs and attach photographs of empty vials and ampoules with labels intact along with the bills for those consumables whose MRP is more than Rs. One thousand only(Rupees only (Rupees 1000/-). ⮚ If hospital is having tie-up with another hospital for services (RT/CT etc.,) Provider shall agree beneficiary will be sent to give patient feedback through multimedia using webcam tied up hospital with a referral form (Annexure – XXXI) ⮚ Oncology claims for chemotherapy to be submitted along with drug carton cover photo showing batch number and xxxx. The provision for live viewing of the patient would be provided to the Trust portalexpiry date.
