Employee Contributions While on Leave. Employees participating in the Authority’s insurance plans (including but not limited to health, dental, vision, prescription drug, and optional life) while on any unpaid leave must make the employee premium (or premium equivalent if self- funded) contributions required to keep such coverages in effect. Employee contributions must be received no later than the 15th of the month for coverage to continue in effect for the following month. Failure to continue monthly payments will result in the termination of the employee’s insurance. If coverage terminates, the employee will be offered the option to participate in the Authority’s Insurance Continuation Program (health coverage only), COBRA, at the employee’s expense.‌
