Employee Eligibility, Waiting Periods and Open Enrollment. In addition to the provisions contained in the Eligibility and Enrollment section of the Evidence of Coverage, the following provisions apply to this Contract: CCSB shall be solely responsible for enrollment and eligibility determinations for eligible Employees and their Dependents, and Blue Shield shall rely upon the accuracy of current eligibility and enrollment information furnished by CCSB.
