Initial-year annual Enhancement for BI4Dynamics products commence on the date of purchase of the BI4Dynamics products. Thereafter, Enhancement may be renewed for successive periods of one year. BI4DYNAMICS tries its best to notify the Licensee through RESELLER of renewal information prior to the date for renewal of Enhancement. Related fees are due and payable annually in advance of commencement of the applicable Enhancement Period. Failure to pay Enhancement fees will result in suspension of Enhancement Services and reinstatement fees will apply. In the event that the Enhancement Plan lapse, have not been renewed or were never originally procured, a Reinstatement Fee shall be assessed upon re-commencement of the Annual Service Pan equal to the amount of the Enhancement fees for the period of lapse plus the annual renewal fee, calculated based on the current Enhancement fee in effect at the time of reinstatement.

Enhancement Plan. Subscription to the Enhancement Plan by Licensee is required at all times during the use of the Software. The Enhancement Plan will provide Licensee with all improvements, modifications, refinements, and added features to the software that are released from time to time. Enhancement Plan. 5.1 The Enhancement Plan consists of the right to ob- tain and use any Major Versions and Service Packs for the Li- censor Software license rights for as specified in this Agree- ment and in the separate Software Agreement. The Enhance- ment Plan does not include any rights to new products or to other solutions. It also does not include installation, imple- mentation or upgrade services and such consultancy tasks can be ordered and agreed via a separate contract either be- tween Licensor and Licensee or between Reseller or Licen- see. 5.2 First year of Enhancement Plan is mandatory. 5.3 Licensor provides the Enhancement Plan on a com- mercially reasonable efforts basis in a way it considers appro- priate at its discretion. However, Licensor does not warrant in any way it will provide Major Versions or Service Packs with the same frequency of MS Dynamics, or with any regular frequency whatsoever. The Licensor Software may not be utilized as expected while Li- censor has not yet released its relevant new Updates and Li- censee has not deployed such Updates yet. 5.4 Licensor will provide Enhancement Plan on the cur- rent product version (i.e. the latest Service Pack or Major Ver- sion) of the Licensor Software until a new version is available, although Licensor reserves the right to discontinue provision of Updates to Licensor Software or earlier versions thereof if and to the extent it deems it necessary. Licensor cannot pro- vide optimal Enhancement Plan to Licensee if Licensee does not use the latest version of MS Dynamics. 5.5 All materials, including, but not limited to the knowledge base, demo’s, samples or contributions provided by Licensor (the “Materials”) by whatever means is either owned by or licensed to Licensor. Licensee may only use those Materials as part of the Enhancement Plan and as long as Licensee is entitled to Enhancement Plan. In no event shall Licensee publish, retransmit, redistribute or otherwise repro- duce any Materials in any format to anyone or use any Mate- rials in any connection with any business or commercial en- terprise, without the express written consent of Licensor.   • Payment Plans Employees covered by the Samaritan Choice medical insurance plan who have outstanding balances that are payable to Samaritan Health Services for in network, covered, and authorized (if medically necessary) services will be provided payment plan offerings upon request from the employee. The request will be made to Patient Financial Services, and may be directed through the Hospital Patient Financial Counselor. Patient Financial Services will work with employees to identify the appropriate payment arrangement based on the employee financial needs/eligibility. Within 120 days from first patient statement, employees must contact Patient Financial Services and identify themselves as a SHS SEIU member and ask for a payment plan arrangement that does not exceed six percent (6%) of their household income. Such requests will be granted using the existing SHS payment options and funding programs. To be eligible for a payment plan, employees must comply with all requirements for establishing appropriate payment options/eligibility, including the completion of a financial assistance application with supporting documentation. Employees who comply with all terms of the payment plan(s) will not be subject to collections or wage garnishment.
