Common use of Enrollment/Effective Date of Coverage Clause in Contracts

Enrollment/Effective Date of Coverage. Except for enrollment forms from those Eligible Employees or their Dependent(s) who meet the requirements for late enrollment as set forth in Section 6.1.3, all enrollment forms must be submitted within the Eligible Employee’s or Dependent’s initial enrollment period or during Employer Group’s Open Enrollment Period. Enrollment forms not received during the initial enrollment period or Open Enrollment Period are subject to rejection by Plan. Employer Group shall provide notice to Eligible Employees of the applicable initial enrollment period and Open Enrollment Periods.
