Extended Health. The Colleges agree during the term of this Agreement, to contribute one hundred per cent (100%) of the present premiums towards the current Extended Health Benefit Plan subject to the eligibility requirements provided under such Plan.

Extended Health. Fifty percent (50%) of the billed premium towards coverage of eligible nurses in the active employ for the Extended Health Care Benefits as provided under the VON National Group Insurance Plan, provided that the balance of the premium is paid by each nurse through payroll deductions.
Extended Health. 7.1.3.1 Default level is: • $25 annual deductible single, couple or family • 100% reimbursement of eligible expenses including semi-private hospital room, • paramedical services at $750 per practitioner per person per year, • private duty nursing at $15,000 per year (after age 65 the lifetime maximum becomes $25,000), • hearing aid maximum at $1,000 in any four consecutive years, • international travel assistance for out-of-Canada coverage, • prescription drugs, and • other costs prescribed by a medical practitioner.
Extended Health. Any teacher whose personal illness extends beyond the period compensated may be granted a leave of absence, upon proper application. Any leave granted shall be without pay. After an extended personal illness, the District may request that the teacher present a statement from the physician certifying the teacher's fitness for return to work.

Extended Health. Following ninety (90) calendar days of service with the company, all employees, including their families, are entitled to benefits under the provisions of the insurance company’s Health Care Plan, with an annual deductible of twenty-five (25) dollars per employee for Single coverage and fifty (50) dollars for Family coverage for all covered expenses except prescription drugs. The prescription drugs portion of the Extended Health Plan will be covered by a generic drug plan card (two (2) cards to be issued per family), provided by the insurance company. Each purchase of prescription drugs under this plan will be subject to a five (5) dollar deductible amount to be paid by the employee at the time of each purchase.
Extended Health. Eligible employees, as defined in Articles 1.02 and 1.04, who so indicate to the Board and the Union in writing shall be covered by a mutually acceptable extended health plan with the Board contributing one hundred percent (100%) of the premium.
