Extended Health Benefits. The extended health benefits coverage for CUPE and Fire will be amended to include:
Extended Health Benefits. (i) The Employer, by means of a policy issued by the insurance company, provides extended health benefits to all eligible faculty members. The monthly premium for this benefit is assumed totally by the Employer for each eligible faculty member, spouse/common-law spouse and his/her dependants. Plan benefits will be paid in accordance with the schedule of benefits listed in the plan and will be subject to the limitations specified in the plan including eligibility requirements.
(ii) Total lifetime coverage maximum is unlimited.
(iii) Re-imbursement level on claims is 95%.
(iv) Hearing Aid benefit claims shall be reimbursed to a maximum of $600.00 every sixty (60) consecutive months.
(v) Vision Care benefit claims shall be reimbursed to a maximum of $500.00 during any twenty-four (24) consecutive month period for lenses and frames or contact lenses or laser eye surgery.
(vi) Eye examinations shall be reimbursed to a maximum benefit of $75.00 during any twenty-four (24) consecutive month period.
(vii) Eligible out-of-province/country medical expenses, excluding hospital charges, shall not exceed $25,000.00 in any twenty-four (24) month period.
(viii) Health and welfare benefit coverage shall cease on the day that an employee’s employment terminates.
Extended Health Benefits. If this Contract ends and a [Member] is Totally Disabled and under a Practitioner’s care, We will extend health benefits for that person under this Contract as explained below. This is done at no cost to the [Member]. We will only extend benefits for a [Member] due to the disabling condition. Any services and supplies must be provided before the extension ends. And what We cover is based on all the terms of this Contract. We do not cover services, supplies or charges due to other conditions. And, We do not cover services, supplies or charges incurred by other family members. The extension ends on the earliest of:
Extended Health Benefits a. The Board shall pay seventy-five per centum (75%) of the premium cost of the Provincial Extended Health Benefit Plan for any teacher who wishes to participate in the Plan.
Extended Health Benefits. 1. Total lifetime coverage level will be unlimited.
2. Hearing aid benefit claims will be to a maximum of $600 every five years.
3. Vision care benefit claims will be to a maximum of $250 every two years.
Extended Health Benefits. The Employer shall pay one hundred percent (100%) of the premium cost of the Provincial Extended Health Benefit Plan.
Extended Health Benefits. The Employer, by means of a policy issued by the insurance company, provides extended health benefits, including an eyeglass and contact lens option, to all regular employees and their dependants. The Employer shall pay the full cost of the premiums. Benefits will be paid in accordance with the schedule of benefits listed in the Plan and will be subject to limitations specified in the Plan.
(a) Employees will be reimbursed a total of $75 every two years for vision exams.
Extended Health Benefits. (a) - Group Life Insurance; and
Extended Health Benefits. Members electing to work beyond the TRD will continue to have access to the University’s Employee Assistance Program. MSP premiums and Dental and Extended Health Benefits (EHB), including Vision Care, shall continue (under the terms available to other Members) to be in effect from the TRD to the Member’s actual retirement date. However, out-of-Canada coverage will be removed and an annual maximum of ten thousand dollars ($10,000) will apply to the utilization of EHB from the TRD onward.
Extended Health Benefits. (See Common Agreement, Article 9.2.1(b)) The College will pay 100% of the cost of monthly premiums for employees employed half-time or more. Extended Health coverage shall be in accordance with the terms of the contract between the College and the insurance carrier and shall include:
a) 95% reimbursement on the first $1,000 in-province eligible expenses in excess of the $25 per family deductible and 100% thereafter in the same year.
b) The lifetime maximum amount of benefits payable for any one member or dependent shall be unlimited.
c) Vision care charges for the purchase of corrective lenses and frames or contact lenses. The maximum benefit payable shall be $500 per person in 2 calendar years.
d) One eye examination every 2 years to a maximum of $75.
e) Hearing aids in the amount of $600 every 4 years for any one member or dependent.
