Common use of Follow-up Services Clause in Contracts

Follow-up Services. Follow-up services shall be made available, on an as needed basis for up to twelve (12) months for all of the adults and dislocated workers who are enrolled and placed into unsubsidized employment. The intensity of appropriate follow-up services may vary among customers. Follow-up services may include but are not limited to: additional career planning and counseling; contact with the customer’s employer, including assistance with work-related problems that may arise; peer support groups; information about additional educational opportunities, and referral to supportive services available in the community. The goal of these services is to ensure job retention, wage gains, and career progress for customers who have been referred to unsubsidized employment. Follow-up services must include more than only a contact attempt, or a contact attempt made for the purpose of securing documentation in order to report a performance outcome. Each contact shall be documented in the participant’s file. Retention and follow up services are pivotal to their success and the attainment of performance.
