HEALTH CARE APPOINTMENT. 333T. An employee will be granted time off for health care appointments if requested no later than during the schedule request period. The supervisor may seek verification of the health appointment In order to be considered for time off once the schedule has been posted the employee must give the employee's immediate supervisor written notice and receive approval at least five (5) calendar days prior to the appointment. In the event that a health care professional schedules a return appointment or care, which prevents giving the required notice, as much notice as possible based on the circumstances is required.
