Health Visitors. All families with children under 4 years of age who live within a designated Flying Start area, should be designated to the caseload of a Flying Start Health Visitor. Health Visitors will empower and support children, individuals, families and communities to reach their potential and achieve their fullest health and wellbeing. Health Visitors will build a therapeutic relationship with families and work in partnership with them to assess needs. They have a key role and responsibility for promoting other entitlements and services offered by the Programme and in encouraging and supporting families to take up their full entitlements. They also have a key role in coordinating an integrated approach to the delivery of the entitlements by the Flying Start team to children and their families. The Flying Start Health programme, aligned to the Healthy Child Wales Programme, will be implemented to provide the required enhanced services. From the antenatal stage to the child’s 4th birthday. The Health Visitor will ensure new parents and parents moving into Flying Start areas complete a Flying Start registration form to enable effective information sharing within the programme and are given an information pack explaining the Flying Start programme and their entitlements. To promote Public Health activity within Flying Start areas providing support and advice on healthy lifestyles, infant nutrition, including breastfeeding, weaning and toddler diet; maternal mental health including postnatal depression; immunisations; dental health; and child safety, issues around smoking, substance misuse, alcohol, nutrition, exercise and emotional health . The Flying Start Health Visitor will act as the ‘key worker’ for the family in an attempt to ensure that effective communication channels are developed and maintained between Flying Start service providers based in the Flying Start settings and/or elsewhere, and including other involved agencies so that an integrated service delivery approach is offered to families and identified needs are met. The Health Visitor will undertake a universal comprehensive assessment of health needs and vulnerability alongside the families’ protective factors, to determine whether they are low medium or high risk and undertake regular periodic reassessments. This determines the level of support and interventions required in order to address the needs for the best possible outcomes for the child, which will ensure that those families identified with the greatest need are referred appropriately and engagement in local services is facilitated. The Family Assessment Tool or all Wales resilience assessment tool (FRAIT) will be used alongside professional knowledge and judgement and Health Visitor staff will be trained in its use. Following assessment tailored interventions/referrals/ health promotion activities will be offered at home or in groups to meet the needs of the child and family. The Flying Start Health Visitor will undertake universal developmental assessments of children to enable early identification of developmental issues, and ensure early coordinated intervention will be carried out in line with the Healthy Child Wales programme at 6 months, 15 months, 27 months and a review at 3.5 years. Where indicated additional targeted comprehensive developmental assessments will be undertaken using the Schedule of Growing Skills. This will be used to enable appropriate referral and additional support for the child. Completed Schedule of Growing Skills profiles will be forwarded to the relevant Childcare Setting Leader to facilitate Childcare and Foundation Phase transitions. Children identified with Additional Needs should be raised at Multi Professional Panel meetings to ensure co-ordinated early intervention is put in place in partnership with the parents/carers. Designated Health Visitors attend and contribute to Multi Professional Panel Meetings, Childcare Admission Meetings, Area Meetings and Childcare Transition Meetings. Facilitate group health activities and /or co-facilitate groups with partner agencies in the Flying Start areas. Hold a ‘Child Health Well Baby drop in clinic’ for Flying Start clients within each Flying Start area for growth monitoring, anticipatory guidance and promotion of immunisations. Supporting parents to enable all children to have completed all routine immunisations by 4 years of age. Promote the importance of attachment and support for the development of positive parenting skills with individual families, helping to establish positive relationships between parent/carers and their children. Work in partnership with parents and other agencies and refer to parenting groups. Promote the importance of interaction and communication between parents/carers and their child at all stages of their child’s development. Identify needs and enable intensive early intervention support for children requiring additional input surrounding speech and communication. To follow the agreement with Family Support Programme to act a lead worker for families on the Flying Start caseload when deemed the most appropriate worker to do so. Support all Flying Start staff with child protection procedures including referrals, report writing, court appearance etc. Support all Flying Start staff with child protection training and updates to include domestic abuse issues, working in partnership with safeguarding leads in partner agencies where appropriate. Support all Flying Start staff with supervision regarding vulnerable families. To support all Flying Start Staff and where necessary take on the lead worker role, regarding challenges with partner agencies in the provision of Child Protection support. To attend multi-agency risk assessment conferences on behalf of Flying Start as appropriate. To facilitate attendance and represent Health Flying start at Case Conferences. To facilitate ongoing support of vulnerable families in vacant caseloads through delegation or provision of that support as appropriate. To facilitate a weekly drop-in support session for all staff to access vulnerability support i.e. face to face, phone or email. Group supervision to be provided for Community Health Nursery Nurses.
