Common use of Home Health Care Plan Clause in Contracts

Home Health Care Plan. Shall be defined as a program of Home Health Care established and approved in writing by the Covered Person’s Physician for the provision of Home Health Care Services. The Physician shall state that confinement to a Hospital or Skilled Nursing Facility would be Medically Necessary for the treatment of the Covered Person’s Injury or Illness if the Home Health Care Plan is not provided.
