Hospice Eligibility Period. The hospice eligibility period begins on the first date hospice care services are rendered and terminates one hundred eighty (180) days later or upon the death of the terminally ill Member, if sooner. If the Member requires an extension of the eligibility period, the Member or the Member’s representative must notify CareFirst BlueChoice in advance to request an extension of SAMPLE benefits. CareFirst BlueChoice reserves the right to extend the eligibility period on an individual case basis if CareFirst BlueChoice determines that the Member’s prognosis and continued need for services are consistent with a program of hospice care services.
