The Employer agrees to pay one hundred percent (100%) of the premiums for the following benefits: Pay Direct Managed Drug Plan-Assure Health's National Formulary: 95% of all eligible drugs, medicines and applicable pharmacy dispensing fees- $11.99 75% for all other non-formulary drugs which require a prescription.
