INTERVENTION PROTOCOL. MONTH 1: Patient enrollmenet via business reply mailer - Initial telephone intervention within seven to ten days of business reply mailer receipt - Patients receive demand published report following IVR intervention - Appropriate educational materials sent to patients - Physician receives demand published, patient specific report for inclusion in patient record following IVR intervention - Additionally physician receives immediate notification via phone/fax for any patient reporting status indicating a critical "at risk" trigger has been met - Patients receive interactive voice response based telephone interventions monthly - Patients receive demand published report following each IVR intervention - Appropriate educational materials sent to patients - Physician receives demand published, patient specific report for inclusion in patient record following IVR intervention at month 2 and 4 - Additionally physician receives immediate notification via phone/fax for any patient reporting status indicating a critical "at risk" trigger has been met MONTHS 6 AND 8 - Patients receive interactive voice response based telephone interventions - Patients receive demand published report following each IVR intervention - Appropriate educational materials sent to patients - Physician receives demand published, patient specific report for inclusion in patient record following IVR intervention at month 8 - Additionally physician receives immediate notification via phone/fax for any patient reporting status indicating a critical "at risk" trigger has been met PROGRAM INTERVENTION DESCRIPTION

  • Improvement Plan A detailed, written plan initiated by the evaluator. The teacher may provide input at the meeting to review the plan. Improvement plans are utilized when a teacher receives an Evaluation Rating of Ineffective or when an administrator utilizes discretion to place any teacher on an improvement plan at any time based on any individual deficiency in the evaluation system. The approved form for the Improvement Plan is attached to this agreement as Appendix A-9.

  • Signaling protocol The Parties will interconnect their networks using SS7 signaling where Technically Feasible and available as defined in GR 905 Telcordia Standards including ISDN User Part (ISUP) for trunk signaling and TCAP for CCS-based features in the Interconnection of their networks. All Network Operations Forum (NOF) adopted standards shall be adhered to. Where available, CenturyLink signaling services to link its Signaling Transfer Points (STPs) for CLEC switches which connect to CenturyLink’s STPs via “A” links or for CLEC’s STPs to connect to CenturyLink’s STPs via “D” links which are dedicated to the transport of signaling for local Interconnection, may be ordered from the CenturyLink Tariff.
