Last Day Requirement. The Participant is an Employee of the Employer on the last day of the Plan Year. However, this condition will be waived for the following reasons (Check at least one): [X] The Participant's Death. [X] The Participant's Termination of Employment after having incurred a Disability. [X] The Participant's Termination of Employment after having reached Normal Retirement Age. [ ] This condition will not be waived.
Last Day Requirement. The Participant is an Employee of the Employer on the last day of the Plan Year.
Last Day Requirement. The Employer may elect in its Adoption Agreement to require a Participant to be employed by the Employer on the last day of the Plan Year or other specified period or on a specified date.

Decrease program costs when possible by procuring items at no cost from County surplus stores and by accepting delivery of such items by County.

  • Federal Medicaid System Security Requirements Compliance Party shall provide a security plan, risk assessment, and security controls review document within three months of the start date of this Agreement (and update it annually thereafter) in order to support audit compliance with 45 CFR 95.621 subpart F, ADP System Security Requirements and Review Process.
