Maintain Qualification Standards. Provider shall have and maintain the appropriate Medicare certification and licensure under applicable federal or state laws to conduct business, and Provider shall meet and maintain all requirements in the BCBSM Qualification Standards as set forth in Addendum B. Upon request, Provider will submit to BCBSM evidence of continuing compliance with the Qualification Standards, and shall promptly notify BCBSM in writing if Provider no longer meets the Qualification Standards.
