Mandatory Forms. In addition to this information paper, all Cadets must complete two mandatory forms to participate in JROTC. They are the JROTC Parental/Guardian Agreement / Statement of Health and the JROTC Cadet Personal Data Sheet. One optional form is the Parent Permission Slip.

Mandatory Forms. Party A requires Party B to complete the KYC and applicable FATCA forms and submit to Party A for onboarding Party B as a counterparty. The onboarding process must be completed and approved by Party A before any trading may commence. These two forms shall be updated upon unilateral request by Party A. Party A may request updated forms in the event that Party B has organizational changes, change of ownership, acquisition, merger or other change. Otherwise, Party A will require the forms to be updated every three years, whichever occurs first. Both forms and its updates shall be send by Party B (if a Direct Buyer) or by Party C (if Party B is not a Direct Buyer) to ACYRSA via email or any other communication means approved by ACYRSA, such as the Extranet. ACYRSA will provide these forms to Party A through the Extranet. Party A shall perform the onboarding and compliance checks and will only be able to send to Party B or C the Confirmations for each uploaded Trade as described in (c) above, if the KYC/ FATCA checks were successful.

  • Payment Authorization and Payment Remittance By providing the Service with names and account information of Billers to whom you wish to direct payments, you authorize the Service to follow the Payment Instructions that it receives through the Site. In order to process payments more efficiently and effectively, the Service may edit or alter payment data or data formats in accordance with Xxxxxx directives. When the Service receives a Payment Instruction, you authorize the Service to debit your Eligible Transaction Account and remit funds on your behalf so that the funds arrive as close as reasonably possible to the Scheduled Payment Date designated by you. You also authorize the Service to credit your Eligible Transaction Account for payments returned to the Service by the United States Postal Service or Xxxxxx, or payments remitted to you on behalf of another authorized user of the Service. The Service will attempt to make all your payments properly. However, the Service shall incur no liability and any Service Guarantee (as described in Section 3 of the Bill Payment Terms) shall be void if the Service is unable to complete any payments initiated by you because of the existence of any one or more of the following circumstances:
