Exhibits and Forms Manuals, reports, exhibits and forms referenced in this Contract are located at xxxxx://xxx.xxxxx.xxx/doing-business-hhs/provider-portals/behavioral-health-services- providers/behavioral-health-provider-resources/community-mental-health-contracts. Performing Agency shall use the reports and forms required by this Contract as they now exist and as they may be revised. System Agency will notify Performing Agency of revisions to the reports and forms. All manuals, reports, exhibits and forms are incorporated by reference into this contract.
