Common use of Medical and Group Life Insurance Clause in Contracts

Medical and Group Life Insurance. The Company agrees to include Executive only in the group medical and hospital plan of the Company, and provide group life insurance for Executive only at no charge, in the coverage amount of $15,000.00,(Fifteen Thousand Only) during this Agreement. Family members may be included in group medical and hospital plan at normal family rates. Executive shall be responsible for payment of any federal or state income tax imposed upon these benefits.

Medical and Group Life Insurance. The Company agrees to include the Executive only in the group medical and hospital plan of the Company, Company and provide group life insurance for the Executive only at no charge, charge to the Executive in the coverage amount of $15,000.00,(Fifteen Thousand Only) equal to three times base salary during this Agreement. Family members may be included in group medical and hospital plan at normal family rates. The Executive shall be responsible for payment of any federal or state income tax imposed upon these benefits.

Medical and Group Life Insurance. The Company agrees to include Executive only and his family members in the group medical and hospital plan of the Company, Company and provide group life insurance for Executive only at no chargecharge to the Executive, in the coverage amount of $15,000.00,(Fifteen Thousand Only) during this Agreement. Family members may be included 5,000,000 payable to the Company and another policy in group medical and hospital plan at normal family ratesthe amount of $2,000,000 payable to the Executive's estate. Executive shall be responsible for payment of any state or federal or state income tax imposed upon these benefits. If the Company does not provide Medical and Group Life insurance the Executive may be reimbursed for his own coverage.

Family members may be included in group medical and hospital plan at normal family rates. Executive shall be responsible for payment of any state or federal or state income tax imposed upon these benefits.

Medical and Group Life Insurance. The Company agrees to include Executive only in the group medical and hospital plan of the Company, Company and provide group life insurance for Executive only at no charge, charge to Executive in the coverage amount of $15,000.00,(Fifteen Thousand Only) the cost to cover Executive’s entire family under a Grade A PPO medical and dental program during this Agreement. Family members may be included in group medical and hospital plan at normal family rates. Executive shall be responsible for payment of any federal or state income tax imposed upon these benefits.

Executive shall be responsible for payment of any state or federal or state income tax imposed upon these benefits.
