Medical and Dental Insurance. The Company shall pay Employee’s monthly Medical and Dental Insurance premiums in association with Company provided health insurance plans.
Medical and Dental Insurance. Section 21.1 The Employer will contribute toward full-time Employee and family health insurance premiums in the amounts as determined by the City-wide benefits committee which shall include the President of AFSCME Local 1180 and one (1) additional representative selected by AFSCME Local 1180. Such contributions shall be prorated for new hires, so that new hires and current employees pay the same biweekly rate.
Section 21.2 Employees may only change elections during open enrollment periods and certain life events as designated by the Employer and as required by law.
Medical and Dental Insurance. The Executive and his eligible dependents who participate in these benefits prior to termination may elect COBRA benefits and the Company will adjust the COBRA payment for the entire 18 months of COBRA to be equal to what the participant would have contributed as an active employee.
Medical and Dental Insurance. Executive, his spouse, and his dependents, shall be entitled to participate in such medical and dental insurance programs as are provided to the Corporation’s other executive employees.
Medical and Dental Insurance. In the event that the Company, with the approval of the Board of Directors, elects to establish a Medical Insurance Benefit Plan for the benefit of the Company’s employment staff, Employee shall be entitled to participate in such plan which shall include comprehensive medical and dental insurance (from a reputable and financially-sound insurance carrier of national standing) for himself and his immediate family. Such insurance shall cover at the minimum 100% of all hospitalization costs after payment of deductibles and 80% of other medical costs, with the annual deductible not exceeding $500 per person. There shall be no cap on benefits for the medical insurance, and the annual cap for dental insurance benefits shall not be less than $3,000. The Company may either provide these benefits directly to Employee or promptly reimburse Employee for the cost of such benefits, at the Company’s election.
Medical and Dental Insurance. UNIFI will continue to provide EMPLOYEE medical and dental coverage similar to the medical and dental coverages at that time being provided to regular employees covered by the terms of the Unifi, Inc. Employee Welfare Benefit Plan then in effect (the "Medical Plan"), until the earlier of the last day of the month in which EMPLOYEE attains the age of 65 years or until such time as EMPLOYEE has began new employment, including gainful self-employment (as determined by Unifi in its sole discretion). EMPLOYEE shall be eligible to receive such medical and dental benefits in order that he may obtain coverage for himself and his dependents, as the term "dependent" is defined in the medical plan, so that the following shall apply to coverage of EMPLOYEE and his dependents.
(A) As a condition of coverage of EMPLOYEE, he must pay for each month of coverage an amount equal to the premium paid for such month by an active employee for coverage under the Medical Plan. During the time when Monthly Payments, if any, are being made, such premiums shall be paid by deductions from such installments unless UNIFI in its sole discretion determines otherwise. Thereafter, such premiums shall be due on the first day of the month to which they apply, and the medical and dental coverage shall be terminated unless such premiums are received when due, without any grace period.
(B) As a condition of coverage of a Dependent, EMPLOYEE must pay for each month of coverage an amount equal to the premium paid for such month by any active employee for coverage of a Dependent under the Medical Plan. (C) The terms of medical and dental coverage for EMPLOYEE and his Dependents at any given time shall be the terms applicable to active employees and their Dependents at such time. It is explicitly understood and agreed that any amendments to or alteration of the Medical Plan (including any amendment terminating the Medical Plan) may be applicable to EMPLOYEE and his Dependents without regard to whether the amendment or alteration was adopted or made before or after the Effective Date, and/or the date EMPL...
Medical and Dental Insurance. Medical and Dental insurance plans may be continued in accordance with the Federal Consolidated Omnibus Budget Reconciliation Act of 1985 or COBRA which generally allows for continuation of insurance coverage for eighteen (18) months beyond the date of insurance termination. The teacher will be responsible for full payment of group rates for continuation of coverage for themselves and eligible dependents. A teacher will have sixty (60) days from date of benefits termination to elect coverage.
Medical and Dental Insurance. During the Employment Period, Executive and Executive's immediate family shall be entitled to participate in such medical and dental benefit plans as the Employer shall maintain from time to time for the benefit of senior executive officers of the Employer and their families, on the terms and subject to the conditions set forth in such plans.
Medical and Dental Insurance. If Executive elects to continue his current medical and dental family coverage under Company’s Medical and Dental Plans, the Company shall pay for the Company’s portion of the premiums for the period beginning on the date of termination of employment through the date payments to Executive cease pursuant to Section 8(d)(iii) hereof and the Executive shall pay for the Executive’s then-current portion of the premiums for said coverage. Notwithstanding the above, at such time as Executive secures new employment providing medical and dental coverage, Executive shall promptly notify Company and the Company’s obligation to pay the Company’s portion of the premiums for coverage will cease. At the end of the applicable period, Executive may elect to continue such COBRA coverage for the applicable COBRA period, at his own expense, in accordance with the terms of the Company’s Medical and Dental Plans.
Medical and Dental Insurance. The Board will continue to provide a medical, vision, prescription drug, dental, life and long-term disability insurance plan, which will be in effect for the 2022- 2026 school years. Contributions will be paid by the Board for employees who work a minimum of thirty (30) hours per week. Insurance contributions are based on 24 pay periods. For each school year, the total projected cost of the comprehensive medical, vision, prescription drug and dental plan, including anticipated claims, third- party administrative expenses, stop-loss premiums, hospital/physician network administration, life insurance premiums, long-term disability premiums, accidental death and disability premiums, anticipated employee assistance and wellness costs, shall be divided between the Board and the classified bargaining unit, with the Board paying 83% and the classified staff paying 17%. The specific allocation between the participant categories and the estimate of projected/anticipated claims and other cost items for which actual premiums are not then available, shall be determined by the Administration based upon recommendation from the District Insurance Committee as soon after October 30 as the prior year data is available. After the District Insurance Committee has met and any plan changes approved by the Association and the Board have been incorporated, employee premium contributions shall be established by the Administration based on projected costs.
A. Insurance coverage is optional. New employees may elect not to participate in the district’s health and dental insurance plans upon written notice to the Business Office during the enrollment period. Those employees declining medical and dental insurance will continue to receive term life insurance coverage ($50,000), long-term disability insurance coverage and accidental death and dismemberment insurance coverage at Board expense.
B. District married couples will receive one family plan coverage at the cost of two single contributions based on the most senior employee’s year of service.
C. There will be no change in employee coverage – single to family or vice versa – unless there has been a change in status, i.e., marital, employment, birth, adoption.
D. For the duration of the 2022-2026 Contract, employees who elect and maintain insurance coverage will have the annual option of selecting to participate in a Value Health Savings Account Plan (High Deductible Health Plan) or a Preferred Provider Organization (PPO) Plan.
