Common use of Medical Statements Clause in Contracts

Medical Statements. 35 At the discretion of the Agency, an employee may be required to provide reasonable evidence 36 (i.e., a physician certification) to the Human Resources Office that confirms the medical necessity 37 for illness/disability leave. Any expense involved in providing such medical evidence shall be the 38 responsibility of the employee. In the event a required certification with sufficient information is 39 not received in the Human Resources Office by the timeline for when the evidence is to be 40 provided, paid leave is not available for the period of time the needed evidence was not 41 presented. 43 Employees absent for multiple days for medical reasons may be required to file a medical 44 statement with the Human Resources Office before returning to work. This statement must verify 45 that the employee is able to adequately perform assigned job responsibilities.
