Common use of Member Identification Cards; Verification Clause in Contracts

Member Identification Cards; Verification. QualCare shall assist Payor in providing each of its Members with an identification card that indicates his/her enrollment in Payor’s Health Benefits Plan. This identification card shall include the QualCare logo, a description of the network to be accessed, the applicable telephone number(s) for eligibility, benefits and UM questions, the Payor identification number, and other information as may be required by the applicable Payor.

QualCare shall assist Payor in providing each of its Members Member with an identification card that indicates his/her enrollment in Payor’s Health Benefits Plan. This identification card shall include the QualCare logo, a description telephone number where Participating Provider may obtain verification of the network to be accessed, such Member’s enrollment in the applicable telephone number(s) for eligibility, benefits Plan and UM questions, the Payor eligibility and range of Covered Services. The Member identification number, and other information card as may be required provided by Payor will clearly identify the applicable PayorQualCare logo on the Member’s identification card.
