Identification Card. Any cards issued to Members are for identification only.
A. Possession of an identification card confers no right to benefits.
B. To be entitled to such benefits, the holder of the card must, in fact, be a Member on whose behalf all applicable Premiums have actually been paid.
C. Any person receiving benefits to which he or she is not then entitled will be liable for the actual cost of such benefits.
Identification Card. The identification card issued by HMO to Members pursuant to this Certificate is for identification purposes only. Possession of an HMO identification card confers no right to services or benefits under this Certificate, and misuse of such identification card may be grounds for termination of Member’s coverage pursuant to the Termination of Coverage section of this Certificate. If the Member who misuses the card is the Subscriber, coverage may be terminated for the Subscriber as well as any of the Covered Dependents. To be eligible for services or benefits under this Certificate, the holder of the card must be a Member on whose behalf all applicable Premium charges under this Certificate have been paid. Any person receiving services or benefits which such person is not entitled to receive pursuant to the provisions of this Certificate shall be charged for such services or benefits at billed charges. If any Member permits the use of the Member’s HMO identification card by any other person, such card may be retained by HMO, and all rights of such Member and their Covered Dependents, if any, pursuant to this Certificate shall be terminated immediately, subject to the Grievance Procedure in this Certificate.
Identification Card.
3.12.8.1 For MSHO, an integrated Medicare and Medicaid identification card, and
3.12.8.2 For MSC+, an identification card that conforms to the requirements in Minnesota Statutes, §62J.60, subd. 3.
3.12.8.3 MSHO and MSC+ cards must be approved by the STATE prior to distribution. The card must identify the Beneficiary as an MCO Enrollee and contain an MCO telephone number to call regarding coverage, procedures, and Grievances and Appeals. The identification card shall demonstrate that the Enrollee is a Beneficiary of MHCP, either by showing the Enrollee’s STATE PMI number on the card, or by other reasonable means. The card may include data elements required by CMS for Medicare eligible Enrollees.
3.12.8.4 The MCO and/or its Pharmacy Benefit Manager Subcontractor must assign a unique BIN/PCN combination that will only be used for MHCP enrollees, including Medical Assistance, MinnesotaCare, and dual eligible integrated programs. The same BIN/PCN combination can be used for all MHCPs. The MCO and/or PBM must not use the same BIN/PCN combination for its commercial or standalone Medicare Part D enrollees. The MCO must provide the unique BIN/PCN combination numbers to the STATE. The identification card containing the unique BIN/PCN combination must be made available to the MCO’s Enrollees.
Identification Card. Cards issued by HPN to Members are for identification only. Possession of the HPN identification card does not give right to services or other benefits under this Plan. To be entitled to such services or benefits, the holder of the card must in fact be a Member and all applicable premiums must actually have been paid. Any person not entitled to receive services or other benefits will be liable for the actual cost of such services or benefits.
Identification Card. Cards issued by SHL to Insureds pursuant to this Plan are for identification only. Possession of an SHL identification card confers no right to services or other benefits under this Plan. To be entitled to such services or benefits the holder of the card must, in fact, be an Insured on whose behalf all applicable premiums under this Plan have actually been paid. Any person receiving services or other benefits to which he is not then entitled pursuant to the provisions of this AOC will be liable for the actual cost of such services or benefits.
Identification Card. (A) The Contractor shall issue an identification card to all Enrollees. The identification card shall contain the following information:
(1) Children’s Health Insurance Program (CHIP);
(2) Whether the Enrollee is Plan B or Plan C, and if they are Native American.
(3) The name of Contractor’s Dental Plan;
(4) A toll free Member Service number.
(B) The Contractor shall issue the identification card to new Enrollees within 21 calendar days after the Department notifies the Contractor that the Enrollee has been enrolled in the Contractor’s Dental Plan.
(C) The Contractor must issue a new identification card to incumbent Enrollees when their coverage terminates for 60 days or more before reinstating with the Contractor’s Dental Plan, when the Enrollee changes Plans (B or C), or when the Enrollee has changes to their co-pay exempt status.
(D) The Contractor is not required to issue an identification card to Enrollees who have qualified for only retroactive enrollment in the Contractor’s Dental Plan.
Identification Card. The Contractor shall submit the identification card to the Department for prior approval initially and as revised. The Contractor shall not be required to submit for prior approval format changes, provided there is no change in the information conveyed.
Identification Card. The Board agrees to provide employees on request with Corporate Identification Cards.
Identification Card. (A) The Contractor shall issue an identification card to all Enrollees. The identification card shall contain the following information:
(1) Children’s Health Insurance Program (CHIP);
(2) Whether the Enrollee is, Plan B or Plan C, and if they are American Indian/Alaska Native. The Contractor will begin including the term American Indian/Alaska Native within 120 days of the start of the contract begin date.
(3) The name of Contractor’s Dental Plan;
(4) A toll free Member Service number.
(B) The Contractor shall issue the identification card to new Enrollees within 21 calendar days after the Department notifies the Contractor that the Enrollee has been enrolled in the Contractor’s Dental Plan.
(C) The Dental Plan must issue a new identification card to incumbent Enrollees when their coverage terminates for 60 days or more before reinstating with the same Dental Plan, when the Enrollee changes Plans (B or C), or when the Enrollee has changes to their co-pay exempt status.
(D) The Dental Plan is not required to issue an identification card to Enrollees who have qualified for only retroactive enrollment in the Contractor’s Dental Plan.
Identification Card. 4.16.1.7.1 Upon initial enrollment of DCFS Youth, the name and phone number of an Enrollee’s PCP is not required.
4.16.1.7.2 For DCFS Youth Enrollees, Contractor shall reissue an identification card to an Enrollee when a PCP is assigned or chosen.
