Notice of Intent to Enroll at LECOM. Within two weeks from the date he or she received notice of full acceptance into the LECOM Doctor of Osteopathic Medicine Program, a successful applicant must notify the LECOM Admissions Coordinator that he or she wishes to enroll. Payment of the $1500.00 matriculation fee is not required at the time the notice to enroll is made, but must be made no later than the first week of June. Students not notified of full acceptance until after the first week of June must pay the matriculation fee when submitting their notice of intent to enroll. The matriculation fee will be applied to the first year’s tuition.

The matriculation deposit fee must be paid within the time frame specified in the letter of acceptance, typically 30-days, in order to ensure their seat isreserved.

  • Notice of Material Breach and Intent to Exclude The parties agree that a material breach of this CIA by Healogics constitutes an independent basis for Healogics’ exclusion from participation in the Federal health care programs. The length of the exclusion shall be in the OIG’s discretion, but not more than five years per material breach. Upon a determination by OIG that Healogics has materially breached this CIA and that exclusion is the appropriate remedy, OIG shall notify Healogics of: (a) Healogics’ material breach; and (b) OIG’s intent to exercise its contractual right to impose exclusion.
